This is two assignments the first assignments is a Capstone Portfolio. I need you put all my papers together use transition sentences when introducing putting together. I provided an example of Capstone. You just add one or two sentences to each of my papers so it can really look like one.
The second is a power point presentation 8/10 is fine, I send over video and pictures.
Here the directions and what they looking for
The capstone paper and presentation will represent the culminating assignment for the student’s field practicum experience. The student will work on different portions of the assignment throughout the year. It is expected that the student gathers information and consult with their agency site supervisor in gathering and analyzing the data.
 
 
Different portions of the assignment will be due periodically during the two semesters (SWK 353 and SWK 354). These sections will be evaluated and be a contributing part of the student’s final grade for that particular semester. It is expected that any evaluative comments made on the individual sections submitted by the student be incorporated into the final paper. Students are reminded of the guidelines in the syllabus in preparing their paper (APA Style) Students must incorporate at least ten references into their Capstone papers and presentations.
The Capstone paper and oral presentation should follow the outline below. The length of the paper cannot be anticipated. It will be evaluated on its thoroughness, structure, content, grammar, and overall professionalism (including appearance).
 
 
The presentation must be 30 minutes in length, including the presentation and discussion. Points will be deducted for poor management of time. The presentation must be clear, engaging, organized, and demonstrative of excellent oral communication skills, including the appropriate use of social work concepts and terms, and person-first language. The student is required to use aids such as power point and handouts. The student’s creativity is essential
Establishing the Context: (Based on paper submitted fall semester in SWK 353)
 
 
The agency and its clients:
 
Describe the auspices of your agency, i.e. public, private – non-profit, proprietary. Discuss the history of your agency.
 
Describe the field of practice
 
Describe the population served by the agency, i.e. socio-economic group, ethnicity, age, etc.
 
Describe the agency’s mission, its organizational structure, and the composition of staff. If the program that the student is working in is part of a larger whole, describe the entire agency and provide the context within the agency of this particular program.
 
Discuss the agency’s funding base.
 
Discuss your field of practice in an international context.
 
 
Relevant social policy (Based on Relevant Social Policy and Cultural Humility Paper submitted fall semester in SWK 353)
 
 
Describe two social policies related to delivery of services at your agency. These policies must be in the form of state/federal legislation and/or case law.
 
How do these policies impact your role as a social worker?
 
How do these policies impact clients?
Diversity and Cultural Humility (Based on Relevant Social Policy and Cultural Humility paper submitted fall semester in SWK 353)
 
 
Describe the agency’s policy or policies on diversity
 
Discuss and evaluate the cultural competence of the staff at the agency. Specifically relate the ethnic composition of the staff in regards to the diversity of their client population.
 
 
Describing the Client: (Due Date: Week Six of Spring Semester-SWK 354)
 
 
Generally, describe the clients served, (i.e. age, gender, ethnicity, other relevant social characteristics).
 
Discuss the route by which the client comes to your agency, (i.e. mandated, self-referral, etc.)
 
The reason for the agency’s work with the client system, (i.e. prevention, problem-solving, etc.)
 
Discuss and describe the network of social service agencies that the agency uses as resources and as referrals. Are the client needs adequately addressed? Are there service gaps?
 
Use of the Planned Change Process with the Client System: (Due Date: Week Nine of Spring Semester-SWK 354)
 
 
Describe the engagement process (including group composition, if appropriate) with a particular client system (individual, family, or group as appropriate for particular site). ALL SOCIAL SERVICES AGENCIES HAVE A CLIENT SYSTEM! The “Client” is the individual, couple, family, group, community, population, and/or demographic for whom your agency exists. Check your agency’s mission statement if you are not sure about the “Client.”
 
 
Assessment of the particular client
 
Identify the presenting problem, issues, and/or concerns.
 
Identify the client’s strengths/resources
 
Identify any and all known barriers or obstacles in the client’s environment or relationships.
 
Present a summary of the bio-psychosocial assessment statement
 
Present a summary of the contract/treatment plan that was established with the client (including goals, task statements, etc.)
 
Intervention
 
Describe the underlying theory (or theories) you have utilized in our work with the client)
 
Why was the particular theory/approach chosen? (i.e. effectiveness, agency policy)
 
Discuss the “middle’ phase of the work with the client. It is anticipated that this part of the presentation will vary greatly among students, depending on the practice setting, modality, and client population.
 
 
Termination and Follow-up
 
Discuss the termination process or the anticipated termination process and plans.
 
 
Evaluation
 
What tools/approaches were used to evaluate outcomes?
 
To what extend did you accomplish your goals with the client?
 
What would you have done differently?
 
 
Ethical Implications of Work with Client System: (Due Date: Week Four of Spring Semester-SWK 354)
What ethical issues tend to arise with the population?
 
Discuss an ethical dilemma that arose during your field placement and the manner in which it was handled.
 
 
Conclusions:
 
 
Draw conclusions about the work in the practicum site.
 
Evaluate your learning experience and describe what you did while at your internship site.
 
Describe and discuss your use of supervision and consultation during your field practicum
 
Discuss what you brought to your site (skills, knowledge, passion, etc., and what you took away)
 
Describe goals for further learning in relation to this particular setting or to this population.
 
Discuss how your courses and experiences as a social work student at NEIU have contributed to your learning experience and what you will “take away” from this collection of experiences.
 
 
CAPSTONE PORTFOLIO:
 
 
Your Capstone Portfolio is a binder that MUST contain the following:
 
 
Your Completed Capstone Paper
 
Your Presentation PowerPoint, Handouts, etc.
 
One “Artifact” from your agency (flyer, newsletter, newspaper article, annual report, etc.)
 
Your current resume that INCLUDES your experience at your field site
 
CAPSTONE PRESENTATION:
 
 
Your Capstone Presentation Must:
 
 
Be no longer than 30 Minutes (Including set-up, discussion, and questions)
 
Contain the highlights of your Capstone Paper (please do not read your paper)
 
Be Creative, Engaging, and Professional
 
Contain a description of your role, tasks, and achievements at the agency (what did you do, and accomplish while at your agency)
 
Include a PowerPoint/Prezi/SNAP Presentation. Handouts are optional…but encouraged
 
Include a discussion of how you utilized supervision and consultation, what you brought to your internship, and to the social work program…and what you take away.
